Innovación en la evaluación cuantitativa de la limpieza de los túbulos dentinarios en retratamientos endodónticos. Estudio con MEB
Abstract
OBJETIVO: Evaluar la limpieza de los túbulos dentinarios mediante el diseño y utilización de un innovadormétodo cuantitativo de medición, en casos de retratamiento endodóntico.MÉTODOS: Cien dientes humanos premolares inferiores unicanaliculares (n=100), fueron seccionados ensu porción coronaria y se trataron endodónticamente con el sistema MTwo. Posteriormente se obturaroncon conos de gutapercha y sellador AhPlus (Dentsply/Maillefer- Suiza) mediante compactación lateral.Luego de su conservación, se desobturaron mediante el Sistema MTwo-Retreatment y se distribuyeronaleatoriamente en tres grupos: G1: Irrigación manual (n40), G2: Irrigación ultrasónica pasiva (n40) y G3:GControl (n20). Cada grupo se dividió en dos subgrupos (n20): SG1: Irrigación con NaClO/EDTA y SG2:Lavado con Endosolv R. GControl: Irrigación con Solución Fisiológica. Cada muestra se seccionó endirección bucal-lingual para visualizar los túbulos dentinarios utilizando MEB (Carl Zeiss Sigma, Alemania).Para determinar la densidad de los túbulos dentinarios abiertos se propuso un método innovador utilizandoel software Image Pro-Plus V5 (Media Cybernetics Inc, EEUU). Se aplicaron los modelos de ANOVA deMedidas Repetidas (p<0.05) y Regresión Lineal Generalizada. RESULTADOS: Los porcentajes deobstrucción tubular relativa fueron SG1.1: 18.38%, SG 1.2: 34.16%, SG 2.1: 25.6% y SG 2.2: 43.6%. Seencontraron diferencias significativas al comparar las soluciones en SG1.1 vs SG 1.2 (p=0.002), así comoen el SG 2.1 vs SG 2.2 (p=0.003). No se encontraron diferencias significativas al comparar los sistemas deirrigación: SG 1.1 vs SG 2.1 (p=0.264) y SG 1.2 vs SG 2.2 (p=0.107).CONCLUSIONES: La composición química del irrigante influyó más en la limpieza de los túbulosdentinarios que el sistema de irrigación utilizado.
OBJECTIVE: Evaluate the cleaning of the dentinal tubules through the design and use of an innovative quantitative method of measurement, in endodontic retreatments.
METHODS: One hundred single-canalicular mandibular premolar teeth (n=100) were sectioned in their coronal portion and treated endodontically with the MTwo system. Subsequently were obturated with gutta-percha cones and AhPlus sealer (Dentsply/Maillefer-Switzerland) using of lateral compaction. After conservation, the obturation was removed using the MTwo-Retreatment System and randomly distributed into three groups G1: Manual irrigation (n40), G2: Passive ultrasonic irrigation (n40) and G3: Control (n20). Each group was divided into two subgroups (n20): SG1: Irrigation with NaClO/EDTA and SG2: Washing with Endosolv R. GControl: Irrigation with Saline Solution. Each sample was sectioned in a buccal-lingual direction to visualize the dentinal tubules using SEM (Carl Zeiss Sigma, Germany). To determine the density of open dentinal tubules, an innovative method was proposed using Image Pro-Plus V5 software (Media Cybernetics Inc, USA). The ANOVA of Repeated Measures (p<0.05) was applied and was complemented with Generalized Linear Regression.
RESULTS: The Relative Tubular Obstruction percentages were SG1.1: 18.38%, SG 1.2: 34.16%, SG 2.1: 25.6% y SG 2.2: 43.6%. Significant differences were found when comparing the effect of the solutions SG1.1 vs SG 1.2 (p=0.002), as well as SG 2.1 vs SG 2.2 (p=0.003). No significant differences were found when comparing the effect of the irrigation systems: SG 1.1 vs SG 2.1 (p=0.264) and SG 1.2 vs SG 2.2 (p=0.107).
CONCLUSIONS: The chemical composition of the irrigant had more influence on the cleaning of the dentinal tubules than the irrigation system used.
