dc.description.abstractObjetivo: Evaluar patrones alimentarios en mujeres lactantes de Córdoba (Argentina) y su relación con sus características sanitarias, ingesta de polifenoles y composición láctea. Métodos: Se realizó un estudio observacional, correlacional y de corte transversal en una muestra de 279 mujeres lactantes dentro de los primeros seis meses postparto, residentes de Córdoba, Argentina. Las participantes respondieron un cuestionario sobre sus características demográficas (edad, nivel educativo, ocupación); reproductivas (posparto, paridad, práctica de lactancia); sanitarias (antecedentes familiares de enfermedades cardiometabólicas - patologías cardiovasculares, hiperlipidemias, diabetes y obesidad-, consumo de tabaco y práctica de ejercicio físico) y antropométricas (porcentaje de grasa corporal -PGC- e índice de masa corporal -IMC-). Además, brindaron una muestra de su leche, la cual fue analizada espectrofotométricamente para determinar la concentración de nutrientes (proteínas triglicéridos y glucosa); oxidantes (hidroperóxidos -HP-, lipoperóxidos -LP- y anión superóxido -AS-) y la actividad de la enzima gamma glutamil-transpeptidasa (GGT). Se determinaron los PA por análisis factorial de componentes principales (AFCP) y se cuantificó el consumo de polifenoles dietarios. Luego, se realizaron modelos multivariados y bivariados para establecer las relaciones entre la adherencia a los PA y las demás variables. Resultados: El AFCP reveló tres PA: patrón macronutricional (PM) compuesto por carne procesada, legumbres, productos lácteos, quesos, granos integrales, golosinas y productos de copetín. Patrón fitoquímico (PF) integrado por vegetales tipo A y B, frutas (frescas), frutas grasas y semillas, carga factorial negativa para café y té. Y patrón energético (PE) constituido por vegetales tipo C, granos refinados, bebidas azucaradas, grasas animales y aceites vegetales. Tener antecedentes familiares de enfermedades cardiometabólicas y realizar ejercicio físico se asociaron con mayor adherencia a PM (OR = 2,69 y 2,68, respectivamente). Las mujeres sin empleo formal tuvieron menor adherencia a PF (OR = 0,49). Un bajo nivel educacional incrementó la adherencia a PE (OR = 2,20). El IMC se asoció inversamente a PF (rho = -0,16). El consumo total de polifenoles alimentarios fue 2404,36 (4653,73) mg/día. La familia de no flavonoides fue la más consumida -500,89 (3575,03) mg/día-, seguida por los flavanoles -210,82 (268,50) mg/día- y los flavonoles -46,53 (148,95) mg/día-. Se encontraron diferencias significativas entre las medias de consumo de polifenoles según el nivel de adherencia a los PA, principalmente en los PM y PF. En este sentido, la adherencia al PF tuvo un gran efecto sobre la ingesta total de polifenoles (d = 1,007). Al analizar los compuestos de la leche humana (LH) se encontró que los promedios de los nutrientes fueron 3,45 (1,83) g/L, 11,17 (6,81) g/L y 30,05 (10,18) g/L para glucosa, proteínas y triglicéridos respectivamente. Además, se halló 7234,39 (3590,97) DO/L de LP, 7254,89 (2962,11) DO/L de HP y 6038,81 (3215,55) DO/L de AS. La actividad de la GGT fue de 4,66 (5,49) UI/L. La concentración de proteínas 11,17(6,81) g/L se asoció positivamente con la adherencia al PM (beta= 0,25, p =0,001). Se halló una relación inversa entre triglicéridos 30,05 (10,18) g/L con la adherencia al PM (beta= -0,25, p =<0,001). Las participantes que adhieren al PM tenían más concentración de anión superóxido DO/L -x (DE) = 6038,81 (3215,55)- en su leche (beta= 133,96, p =<0,001). Por último, se halló que la adherencia al PF se asoció a mayor actividad de la GGT mUI/mg de proteína -x (DE) = 4,66 (5,49)- (beta= 0,26, p =0,006). Conclusiones: Las elecciones alimentarias estuvieron condicionadas por los antecedentes familiares patológicos, el nivel de actividad física, el empleo y el nivel educacional, identificándose tres PA propios de las mujeres lactantes. Además, el nivel de adherencia a dichos PA mostró diferencias en promedio del consumo de polifenoles dietarios y modificaciones en la composición nutricional y oxidativa, así como también en la actividad de la GGT de la LH, por lo tanto, la presente investigación es una contribución para establecer recomendaciones nutricionales a fin de garantizar la adecuada nutrición y salud de las mujeres lactantes y el crecimiento y desarrollo de su descendencia.es
dc.description.abstractObjective: To identify dietary patterns in lactating women from Córdoba (Argentina) and evaluate their relationship with their health characteristics, polyphenol intake and milk composition. Methods: An observational, correlational and cross-sectional study was carried out in a sample of 279 lactating women within the first six months postpartum, residents of Córdoba, Argentina. The participants answered a questionnaire about their demographic characteristics (age, educational level, occupation); reproductive (postpartum, parity, breastfeeding practice); health (family history of cardiometabolic diseases - cardiovascular pathologies, hyperlipidemia, diabetes and obesity -, tobacco consumption and physical exercise) and anthropometric (body fat percentage -PGC- and body mass index -BMI-). In addition, they provided a sample of their milk, which was analyzed spectrophotometrically to determine the concentration of nutrients (proteins, triglycerides and glucose); oxidants (hydroperoxides -HP-, lipoperoxides -LP- and superoxide anion -SA-) and the activity of the enzyme gamma glutamyl-transpeptidase (GGT). Three DP were revealed by principal component factor análisis (PCFA), and the consumption of dietary polyphenols was quantified. Multivariate and bivariate models were performed to establish the relationships between adherence to DP and the other variables. Results: The PCFA revealed three DP: Macronutritional (MP) composed of processed meat, legumes, dairy products, cheeses, whole grains, snack and sweets., Phytochemical (FP) type A and B vegetables, fruits (fresh), fatty fruits and seeds, negative factorial charge for coffee and tea, and Energetic (EP) made up of type C vegetables, refined grains, sugary drinks, animal fats and vegetable oils. Physical activity and family history of cardiometabolic diseases were positively associated with MP adherence (OR = 2.69 and 2.68, respectively). Women without formal employment had lower adherence to FP (OR = 0.49). A low educational level increased adherence to EP (OR = 2.20). Body mass index (BMI) was inversely associated with FP (rho = -0.16). The total consumption of dietary polyphenols was 2404.36 (4653.73) mg / day. The non-flavonoid family was the most consumed -500.89 (3575.03) mg / day-, followed by flavanols -210.82 (268.50) mg / day- and flavanols -46.53 (148, 95) mg / day-. Significant differences were found when comparing the means of polyphenol consumption according to the levels of adherence to the DP, mainly in the MP and FP. Adherence to FP had a great effect on total polyphenol intake (d = 1.007). Respect HM compounds, protein concentration 11.17 (6.81) g / L was positively associated with adherence to MP (beta = 0.25, p = 0.001. The mean of compounds in human milk (LH) were 3.45 (1.83) g / L, 11.17 (6.81) g / L and 30.05 (10.18) g / L for glucose, proteins and triglycerides respectively. Also, 7234.39 (3590.97) DO / L of LP, 7254.89 (2962.11) DO / L of HP and 6038.81 (3215.55) DO / L of AS. The GGT activity was 4.66 (5.49) IU / L. An inverse relationship was found between triglycerides 30.05 (10.18) g / L with adherence to MP (beta = -0.25, p = <0.001). Participants who adhered to MP had a higher concentration of superoxide anion DO / L -x (DE) = 6038.81 (3215.55) - in their milk (beta = 133.96, p = <0.001). Finally, adherence to FP was associated with higher GGT activity mIU / mg protein -x (SD) = 4.66 (5.49) - (beta = 0.26, p = 0.006) Conclusions: Food choices were conditioned by pathological family history, level of physical activity, employment, and educational level. We identified three DP typical of lactating women. In addition, the level of adherence to these DP showed differences in the means of consumption of dietary polyphenols and changes in the macronutrients and oxidant compounds present in HM. Therefore, the present investigation is a contribution to establish nutritional recommendations in order to guarantee adequate nutrition and health of lactating women and the growth and development of their offspring.en
