Expresión de HIF-1 y GAL-3 en Lesiones Blancas Potencialmente Malignas (ESTUDIO PILOTO)

OBJETIVO: El objetivo del estudio fue comparar inmunohistoquímicamente la expresión del Factor 1-alfa inducible por hipoxia (Hif-1) y Galectina-3 (Gal-3) en liquen plano oral (LPO), lesiones liquenoides orales (LLO) y leucoplasia oral (LO). MATERIALES Y MÉTODOS: Se estudiaron 14 cortes histológicos (LPO= 4, LLO y LO= 5) provenientes de tacos de reserva recibidos en el periodo 2015 a 2019, diagnosticados siguiendo los criterios de la Organización Mundial de la Salud (OMS) modificados por Van der Waal y Van der Meij (2003), y con criterios del consenso del año 2020 de Trastornos Potencialmente Malignos Orales (TPMO) mediante un estudio retrospectivo descriptivo (CASI-ODO56I).Se realizó inmunohistoquímica con marcadores citoplasmáticos Hif-1 (1:50), y Gal-3 (1:100) en el laboratorio de Patología Molecular Estomatológica de la Universidad de la República (UDELAR), Uruguay. Ambos marcadores se observaron en toda la extensión de la lámina y se midieron como: negativo (0-5%), positivo tenue (6-49%), y positivo intenso (50-100%). RESULTADOS: La expresión de ambos biomarcadores se observó, a nivel subepitelial para LPO, epitelial para LO (Hif-1 p=0.07 y Gal-3 p=0.08) y epitelial-subepitelial para LLO. Se encontró aumentada la expresión de Gal-3 en LLO y de Hif-1 en LO y pacientes con LPO, que consumían alcohol, presentaron una mayor expresión de Gal-3 (p=0.08).CONCLUSIÓN: Es la primera vez que se estudia la expresión de estos marcadores y podrían predecir la posibilidad de transformación maligna de estos Trastornos Potencialmente Malignos Orales debido al aumento de la neoangiogenesis por acción del Factor 1-alfa inducible por hipoxia y a la Inhibición de la apoptosis como el inicio de la transformación celular por acción de Galectina-3.Estudios con una mayor casuística serán necesarios para esclarecer el rol de estos marcadores en estas patologías
OBJECTIVES: The aim of the study was to compare immunohistochemically the expression of Hypoxia-inducible factor 1-alpha (Hif-1) and Galectin-3 (Gal-3) in oral lichen planus (OLP), oral lichenoid lesions (OLL) and oral leukoplakia (OL). METHODS: Fourteen histological sections (OLP=4, OLL and OL= 5) coming from reserve paraffin blocks, received from 2015 to 2019, diagnosed following the World Health Organization (WHO) criteria modified by Van der Waal and Van der Meij (2003), and from the consensus of oral potentially malignant disorders (OPMD) (2020) were studied through a descriptive retrospective study. Immunohistochemistry was performed with cytoplasmic markers Hif-1 (1:50), and Gal-3 (1:100) in the Laboratory of Stomatological Molecular Pathology of the University of the Republic of Uruguay, UDELAR. Both markers were observed throughout the length of the lamina and were measured as: negative (0-5%), weak positive (6-49%), and strong positive (50-100%). RESULTS: The expression of both biomarkers was observed, at the subepithelial level for OLP, in the epithelial stratum for OL (Hif-1 p=0.07 and Gal-3 p=0.08) and at the epithelial-subepithelial level for OLL. The expression of Gal-3 in OLL and of Hif-1 in OL was found to be increased and patients with OLP, who consumed alcohol, presented a higher expression of Gal-3 (p=0.08). CONCLUSIONS: It is the first time that the expression of these markers has been studied and they could predict the possibility of malignant transformation of these oral potentially malignant disorders due to the increase in neoangiogenesis by the action of Hypoxia-inducible factor 1-alpha and the inhibition of apoptosis as the beginning of cell transformation by the action of Galectin-3. Studies with a greater casuistry will be necessary to clarify the role of these markers in these pathologies.
